Instructions: Please print the form above, fill it out in block letters, and fax it back to
Egypt Panorama Tours at any of the fax numbers above. Please also fax a photocopy of
BOTH sides of your credit card AND the identification page of your passport showing
your signature. We are unable to process a debit to your card without this.

ALSO NOTE THAT CREDIT CARD DEBIT AUTHORIZATION FORMS MUST BE
FILLED OUT IN EGYPTIAN POUNDS (EGP). BECAUSE WE ARE PAID IN EGP
(AS OPPOSED TO FOREIGN CURRENCY), FORMS FILLED OUT IN ANY
CURRENCY OTHER THAN EGP CANNOT BE PROCESSED IN EGYPT.
